My account was shut down last year… When / if at all will I be eligible for a new account from the same IP address, for the same website?

Unfortunately, once kicked out of Adsense you are kicked out for good. You cannot reapply
You may be able to apply if you:
- change your home address
- change your name
- change your website and never put the code on the banned site
- change your web host
- change your ISP
- heck , even change your country
Some are able to reapply, but gets kicked out again before they get their first paycheck because Google finds out that they have been banned before
I suggest you focus your efforts on applying to other ad programs
